USCIS Form I-693
Immigration medical exam
30 min·$290
The immigration medical exam USCIS requires, with civil surgeon Dr. Praveen Saran. Your signed Form I-693 comes back to you in a sealed envelope.
Walk-ins welcome, Monday to Friday, 9 am to 5 pm.
What it covers
Everything the exam includes.
Every immigration medical exam follows the CDC's instructions for civil surgeons.
History and physical exam
An in-person exam of your physical and mental health, with a review of your medical history. USCIS requires civil surgeons to examine you in person.
Vaccination review
Your records are checked against the vaccines USCIS requires: measles, mumps, rubella, polio, tetanus and diphtheria, pertussis, Haemophilus influenzae type b and hepatitis B, plus others the CDC's criteria call for.
A flu vaccine is required from September 1 to March 31. COVID-19 vaccination is no longer required.
TB blood test
An interferon-gamma release assay (IGRA) for everyone 2 and older. A chest X-ray follows only if a result or a symptom calls for it.
Syphilis and gonorrhea tests
A blood test for syphilis from ages 18 to 44, and a urine or swab test for gonorrhea from 18 to 24, or at any age if there's reason to suspect infection.
Your sealed Form I-693
Dr. Saran completes and signs the form and gives it to you in a sealed envelope, with a copy for your records.
Before you come
What to bring.
A photo ID
A valid government-issued photo ID, like an unexpired passport or driver's license. Children under 14 can bring documents showing their name, date and place of birth, and parents' names.

Questions
Questions, answered.
Do I need an appointment?
No. Walk-ins are welcome Monday to Friday, 9 am to 5 pm. You can also book a time online or call (702) 260-6200.
How much does the exam cost?
The exam fee is $290. USCIS doesn't set this fee; each civil surgeon sets their own. Call us if you'd like to ask what your exam will need.
How long is the appointment?
Exam appointments are 30 minutes.
Do I need a COVID-19 vaccine?
No. USCIS doesn't require COVID-19 vaccination records for applications pending on or after January 20, 2025.
What do I do with the sealed envelope?
You submit it to USCIS yourself. If you file by mail, send it unopened with your Form I-485. If you file online, open it, upload the form, and keep the original and the envelope until USCIS decides your case. Don't accept a Form I-693 that isn't sealed.
How long is my Form I-693 valid?
Two years from the date the civil surgeon signs it.
How can I check that Dr. Saran is a civil surgeon?
USCIS lists every designated civil surgeon in its Find a Civil Surgeon tool. Search ZIP code 89102 to see Dr. Saran's listing at this address.
