Read the detailsAn in-person exam of your physical and mental health with Dr. Saran, including a review of your medical history.
Bring the vaccination records you have. They're checked against the vaccines USCIS requires.
A TB blood test for everyone 2 and older, a syphilis test from 18 to 44, and a gonorrhea test from 18 to 24.
Dr. Saran completes and signs your form and gives it to you in a sealed envelope, ready to submit to USCIS.

Dr. Saran graduated from Armed Forces Medical College in 1987 and completed an internal medicine residency at the Medical College of Wisconsin. He holds a Master of Public Health, and patients know him for a compassionate approach and dedication to their care.
